3 May SHELL – Learning from Incidents: IS IT EQUIPMENT THAT REALLY FAILS?

Posted at 21:28h in Safety Bulletins

Many incidents are reported to have stemmed from equipment failures, but IS IT EQUIPMENT THAT REALLY FAILS? What can we do to ensure equipment failures do not occur?

Captains,

Go to the video via the following link. Use the Company Issued Training Laptop and watch the video with your crews. Have the crew participate in the suggested discussion breaks throughout the presentation. Share personal experiences and reflect on lessons learned.

Follow this link to the video:  http://www.rapidview.co.uk/lfi/shell_lfi_equipment/english/story.html

Watch as many times as you would like and provide any constructive feedback or suggestions derived from your discussions to the Cenac Operations and/or Safety and Compliance department.
